MLS Undergraduate Program Handbook

Undergraduate Forms

MLS Professional Program Application Class of 2024-2025: Applications for the 2024-2025 MLS Professional Program will open on September 1, 2023. Information regarding the application and the link to the electronic application will be posted closer to September.  

Application to the Categorical Program/Curriculum: This form is used to apply to the categorical program.

MLS Program Application - Mayo Cohort: This form is used to apply to the Mayo Cohort Program.

MLS Petition Form: Any programmatic decision made by the MLS Professional and Academic Standards Committee (PASC) is eligible for petition if: 1.) the student believes MLS program policy has not been followed, or 2.) if the student has additional information to share beyond what was originally reviewed by the PASC. Students must fill out and submit this form to the MLS Program Director, Brooke Solberg, within 14 days of the PASC decision. 

Department of MLS Pathogen Exposure Management: This form establishes guidelines for managing exposures to bloodborne or biological pathogens within the UND MLS Biohazard department.
