MLS Professional Program Application Class of 2026-2027 :  Applications for the 2026-2027 MLS Professional Program are now open!  Click the link above to begin. Prior to beginning an application, students are encouraged to review the Professional Program Application Guide that we hope is helpful when preparing to apply. Any questions regarding the application or the upcoming cohort year can be directed to our MLS Education Specialist, Katie Kucera. 

Application to the Categorical Program/Curriculum: This form is used to apply to the categorical program.

MLS Program Application - Mayo Cohort: This form is used to apply to the Mayo Cohort Program.

Undergraduate PASC Review Form: The Department of MLS Undergraduate Professional & Academic Standards Committee (PASC) reviews instances where undergraduate students have not met professional and/or academic standards, as outlined in the MLS Undergraduate Handbook. Students involved in an Undergraduate PASC review will be notified in advance and given the opportunity to provide feedback. Submit this form to the MLS Program Chair, Brooke Solberg, to share feedback with the Undergraduate PASC. 

Department of MLS Pathogen Exposure Management: This form establishes guidelines for managing exposures to bloodborne or biological pathogens within the UND MLS Biohazard department.
