Project Request Form
The Office of Communications & Engagement can help support a variety of projects across the School of Medicine & Health Sciences (SMHS). The 'Project Request Form' provides information to our team and helps us plan resources, establish timelines, and support your project effectively.
You will have an opportunity to upload documents, images, or other materials that may help our team better understand your project at the end of the form.

Please note that requests with University of North Dakota (UND) branding are managed by UND Marketing & Communications, including:
- Department/Unit Logos
- Name Badges
- Business Cards
- Email Signatures
Timelines
Whenever possible, submit requests at least 2-3 weeks in advance. Timelines vary and will be determined based on scope, complexity, and current workload. Larger or more complex projects may require additional lead time, and some services may have specific timeline requirements.
The designated point of contact is expected to provide prompt and timely feedback. Failure to do so may result in project delays or cancellations.
Guidelines
Completing the 'Project Request Form' does not guarantee your project will be accepted. If a project is denied you will be provided with a list of pre-approved vendors who may assist with your project.
The Office of Communications & Engagement reserves the right to accept, alter or deny requests based on the scope of the project and staff resources.
