  The Modified Checklist for Autism in Toddlers (M-CHAT) is a validated developmental screening tool for toddlers between 16 and 30 months of age. It is designed to identify children who may benefit from a more thorough developmental and autism evaluation. This is administered at our 18 and 24 month check up.
• Ages & Stages Questionnaires, Third Edition (ASQ-3)
  The ASQ is a screening tool used at check ups from birth to age 5. This form is filled out by parents prior to the visit then scored by nursing staff. It screens for developmental and social-emotional delays. The form is usually ready for you to review just before seeing the patient.

This is a great resource for parents with concerns about vaccines.
